We may use your information to provide patient care and other health-related services. For example, we may automatically enroll you with our telemedicine provider partners and share the information you provide (either directly or through your caregiver) so those providers can deliver services.

Operations:
We may use your information to operate our agency, improve your care, and contact you when necessary. For example, we use health information to manage treatment, services, and internal operations.

Payment:
We may use and disclose your information to bill and receive payment from health plans and other entities. This includes sharing visit data, time in/out, location, photographs, signatures, and caregiver hours worked to properly bill insurance and process payroll.

We only share personal information with third parties as required by law, contract, or as described in this Notice. Service providers act on our behalf, do not independently use the information, and are required to protect it.

Only aggregated, anonymized data may be shared with external providers to improve our Application and services.

We may disclose information for public health and safety purposes when required by law, including:

  • Reporting suspected abuse, neglect, or domestic violence

  • Preventing or reducing serious threats to health or safety

  • Conducting research

  • Complying with laws and regulations

  • Workers’ compensation, law enforcement, and government requests

II. Additional Uses of Information

III. Biometric Information

We may collect biometric information to identify caregivers and patients and verify time entries within our Application.

Biometric information is not disclosed unless:

  • You provide consent

  • The disclosure completes a financial transaction you authorize

  • Required by state or federal law

  • Required by a valid court order, warrant, or subpoena

Biometric data is retained for up to 10 years, after which it is permanently destroyed. We store, transmit, and protect biometric data using safeguards equal to or greater than those used for other confidential information.


IV. Business Transfers

If Rockaway Home Care NV is involved in a merger, acquisition, bankruptcy, dissolution, or asset sale, your information may be transferred as part of that transaction. Any successor organization must honor this Notice.


V. SMS Terms Notification

Rockaway Home Care Nevada SMS notifications provide visit reminders, clock-in/clock-out alerts, document requirements, seminar notices, and other service-related communications.

You may cancel SMS messages at any time by texting STOP. You will receive confirmation of your opt-out. To re-enroll, sign up again as you did originally.

Message and data rates may apply. Message frequency may vary. Carriers are not responsible for delayed or undelivered messages.

No mobile information will be shared with third parties for marketing or promotional purposes. Text message opt-in data and consent will not be shared.


VI. Your Choices

Sharing with Family and Friends

You may choose whether we share information with family members, friends, or others involved in your care, including disaster relief situations.

If you are unable to express your preference, we may share information if it is in your best interest or to prevent serious harm.

Marketing

We do not share your information for marketing purposes without your written authorization.

Fundraising

We may contact you for fundraising, and you may opt out at any time.


VII. Your Rights

You have the right to:

  • Request copies of your medical records (paper or electronic)

  • Request corrections or amendments

  • Request an accounting of disclosures (past 6 years)
