Frequently Asked Questions

Am I eligible for this program In Queens?

You qualify as long as you meet the following criteria: If you are eligible for Medicaid, need nursing home level of care, between the ages 18 and 64 with a physical disability, age 65 or older,  Have a Justification of need for increased services and/or service hours and can live safely in the community with Waiver services then NHTD may be for you.

You Cannot require a skilled level of care at home, only eligible for PCA level of care including assistance with personal care, ADLS, iADLS. You Cannot do more than the aide with the MLTC, and you cannot require  two or more person assistance, since only one aide is approved for each patient.

The two main differences under the waiver program are the approval of increased aide hours to allow for patients that would otherwise require a nursing home to remain at home with aide service and an appointed service coordinator who manages all the patient’s care and coordinates all waiver services.

Explore Queens: The Heart of Diversity in New York City

Queens, one of New York City’s five boroughs, is a captivating blend of vibrant neighborhoods, diverse cultures, and stunning attractions. Known as the world’s most ethnically diverse urban area, Queens offers a unique experience filled with history, cuisine, and breathtaking parks. Whether you’re a local or a visitor, there’s no shortage of reasons to explore this dynamic borough.

History

The story of Queens stretches back centuries, with its lands originally inhabited by Native American tribes such as the Matinecock and Rockaway. Established as one of the original 12 counties of New York in 1683, Queens grew alongside the city, becoming a vital part of the region’s development. From its role as a hub for immigrants in the 20th century to hosting two World’s Fairs in Flushing Meadows-Corona Park, Queens has always been at the forefront of progress and culture.

Must-See Spots

  • Flushing Meadows-Corona Park: Home to the iconic Unisphere, the Queens Museum, New York Hall of Science, Citi Field, and Meadow Lake.
  • Astoria: A vibrant Greek neighborhood known for its food, Museum of the Moving Image, and scenic Astoria Park.
  • Rockaway Beach: Queens’ beach destination for surfing, seafood, and summer fun.
  • Jamaica Bay Wildlife Refuge: A peaceful nature spot ideal for birdwatching and hiking.
  • Long Island City (LIC): Waterfront views, MoMA PS1, and local breweries.
  • Jackson Heights: A global food destination with flavors from India, Colombia, Nepal, and more.

Cultural Vibe

Queens is synonymous with diversity. Over 150 languages are spoken here, making it a melting pot of global traditions and cultures. The borough celebrates this richness through annual festivals such as the Queens Night Market, the Lunar New Year Parade in Flushing, and the Colombian Independence Day Festival.

Venues like Queens Theatre, Flushing Town Hall, and Socrates Sculpture Park showcase the borough's commitment to art and performance.

Local Taste

  • Street Eats in Jackson Heights: Try arepas, momo dumplings, and more.
