You qualify as long as you meet the following criteria: If you are eligible for Medicaid, need nursing home level of care, between the ages 18 and 64 with a physical disability, age 65 or older, Have a Justification of need for increased services and/or service hours and can live safely in the community with Waiver services then NHTD may be for you.
You Cannot require a skilled level of care at home, only eligible for PCA level of care including assistance with personal care, ADLS, iADLS. You Cannot do more than the aide with the MLTC, and you cannot require two or more person assistance, since only one aide is approved for each patient.
The two main differences under the waiver program are the approval of increased aide hours to allow for patients that would otherwise require a nursing home to remain at home with aide service and an appointed service coordinator who manages all the patient’s care and coordinates all waiver services.
