Who Is Rockaway Manor Home Care?

Rockaway Manor Home Care is a non for profit agency licensed by the New York State Department of Health serving Bronx, Brooklyn, Queens, Manhattan, Staten Island and Nassau County. Our mission is clear: to provide the highest quality of care to each and every individual. From the initial point of contact, we are committed to making your home care experience as professional, caring and positive as possible.

At Rockaway Manor Home Care, the pursuit of excellence is our highest priority. We continuously conduct evaluations of service, provide training to staff members and strive to provide our patients with the quality of care they deserve and need.

Every staff member of our agency undergoes complete and comprehensive pre-employment screenings which includes verification of identification and professional references, a full physical examination and proof of ongoing professional training. All administrative and care givers are required take an annual emergency preparedness courses to ensure that they are fully prepared to assist Rockaway Manor Home Care patients in the event of emergencies and disasters.

All caregivers use our automated call in and check out system when arriving and leaving patients homes to keep our office staff up to date in real time with what is going on inside each and every home.

Our Home Care Services

At Rockaway Manor Home Care, we pride ourselves on providing the care you need in the comfort of your own home.


We provide skilled professional nursing care as prescribed by the patient's attending authorizing physician. Services may include, but are not limited to skilled observation and assessment, care planning, paraprofessional supervision, skilled treatments, clinical monitoring and coordination, counseling and referral, wound and dressing care, medication and IV administration and injections.

Home Health Aide

Services including but not limited to personal care, home management and other related support services which are required to allow patients experiencing illness, disability, social disadvantage and other problems to remain in their own home.

Personal Care Aide

Services including but not limited to providing nutritional support, assistance with bathing and daily living activities, providing patients with the ability to remain in their own homes.

Medical Social Work

Patient intervention including psychosocial assessment, financial-environmental assessment, counseling, therapy and community resource planning.

Physical Therapy

Skilled intervention to improve the patient's level of functioning in self-care, mobility, safety, range of motion or strength as well as establishing a safe and effective maintenance program.


Providing patient services including procedures to identify, diagnose, prevent, modify and treat hearing loss as well as any accompanying handicapping conditions.

Occupational Therapy

Providing skilled intervention to improve the patient's upper extremities as well as the fine motor skills necessary to perform functional activities including eating and dressing, in addition to assessing the patient's home for safety and possible modifications in order to improve the patient's ability to function independently.

Respiratory Therapy

Providing instruction to patients and their families about the treatment and maintenance of pulmonary health problems, including the use of respiratory therapy equipment and medications.


Assessing and evaluating the patient's nutritional status as well as providing nutritional counseling and recommendations for improving the patient's dietary regime.

Physician Services

Providing skilled care to patients including examinations, obtaining medical histories, ordering, performing and interpreting diagnostic tests and counseling patients on diet, hygiene and preventive healthcare.


Providing services that will allow patients in need of assistance and direction to maintain a clean and safe environment including but not limited to shopping, cleaning and laundry.


Providing services that will allow patients in need of assistance and direction to maintain a clean and safe environment including but not limited to assistance with meal preparation, care and feeding of well children, shopping, light housekeeping and preparation of household budgets.
